Dear Patients,

We are excited to share the following announcement with all of you. Salem Family Care will be the name of the new medical practice for Drs. Bagwell, Rousher, and Sevilla, effective June 3, 2024. We will be located at 718 East Third Street in Salem. This building is the former location of the Salem Visiting Nurses at the corner of North Lincoln Avenue and Third Street, in Salem.

The Doctors and the staff of Salem Family Care will do everything that we can to make the transition as smooth as possible. Appointments currently scheduled after June 3, 2024, will be at the new location. At Salem Family Care, we are dedicated to putting patient care first. With that in mind, one of our new features is slightly longer appointments, so we can address your concerns at your visit. If your are scheduled during the week of June 3, 2024, you may receive a phone call to adjust the time of your appointment, to help accommodate this new feature. This time adjustment should not be more than 15-30 minutes from your current appointment time.

We will continue to practice at the Family Practice Center of Salem until the end of May. Please look over your current prescriptions and make sure you have enough of your medication so that you do not run out of medication during the transition to our new office, as we will be unavailable to prescibe any medications. Beginning on June 3, 2024, Salem Family Care will be available at the following phone number: 330-639-0880.
