How To Become a New Patient at Salem Family Care
Step One: Contact your insurance company to make sure Salem Family Care is in your network.
Step Two: Stop by the Salem Family Care office with your insurance card and ask for initial paperwork to be come a new patient.
Step Three: After Salem Family Care confirms that we take your insurance, we will contact you.
Step Three: Receive new patient paperwork from the front desk, or click here to download New Patient Paperwork.
Step Four: Before Initial appointment can be scheduled, New Patient paperwork must be completed and turned back into the office.
Step Five: For New Patient appointment, please bring in all medications, vitamins, and supplements you take. Or, you could also bring in a list of your current medications, vitamins, and supplements to the office.
Step Six: Ask Salem Family Care staff about registering for a free patient portal account, and download the free smart phone app for the office.
